Out-of-pocket payments, which are made by individuals, are one of the most important sources of financing for health, especially for low- and lower-middle-income countries. In those countries out-of-pocket payments account for at least half of total health funding. Low-income countries also depend heavily on external funding. As low-income countries transition to middle income, external financing tends to decrease sharply—and is not, for the most part, immediately compensated for by higher domestic public resources. When this happens, people must pay more (out of pocket) to close the gap until public resources increase enough to support health financing and reduce the need for out-of-pocket spending in relative terms.

In absolute terms the gap between rich and poor countries in health spending per capita is staggering. In 2017 high-income countries spent over 60 times more per capita on health than lower-middle-income countries, and 120 times more than low-income economies.
