In my field I am continuously observing clients and patients, bias also comes in, because we often look at history of client / patients which we include in our assessment of establishing a prediction of the diagnosis. We using the SOAP method to establish a more accurate prediction and structuring a report.

SOAP stands for: The SOAP method structures reporting. This not only provides a better overview, but also offers many possibilities for better observation, analysis and, consequently, for making better care plans. Reporting according to the SOAP method is therefore particularly helpful in: rapidly changing circumstances, situations where there is a lot of uncertainty about the causes of a client's behavior, complex client situations.

Subjective: What the client says about his own experiences.

Objective: The direct observation of the situation by the employee. In other words: the client's behavior as perceived by the employee.

Analysis: A conclusion drawn from the subjective and objective data collected by the employee.

Plan: What the employee will do next.

'Google dokter'

There are less people teenagers who are searching on the web for detecting symptoms and diagnoses. The young people generally have fewer complaints compared to the elderly aged 40 and over.

More than 90 percent use the internet to find out what their ailment could mean. Chances are therefore very high that you also use 'Doctor Google'. But it won't make you any healthier.

A few decades ago, before Google existed, we went to the doctor with complaints that were serious or dragged on for a long time. Today we do massive self-diagnosis via the internet.

A recent study at the VU University Amsterdam shows that the more often people look up information about their health online, the more anxious they feel afterwards. The information found worries us even more. People will pay particular attention to the information that confirms that something is supposedly going on. As a result, we are more often concerned, unjustly.

Searching for medical information on the internet can also be a problem if people have little or no medical education, especially when internet searches are used as a diagnostic method as it can increase their anxiety. (cyberchondria)
